The learning disability Transforming Care programme aims to significantly re-shape services for people with learning disabilities and/or autism with a mental health problem, or behaviour that challenges, to ensure that more services are provided in the community and closer to home, rather than in hospital settings. To help achieve this ambition, NHS are establishing five Fast-Track areas across the North of England and Midlands and East. The Fast-Tracks will help shape the transformation of learning disability services more widely across England and will include the future model of care for people with learning disabilities and/or autism.
