Introduction
Empowering patients and putting people in control are important themes in NHS England’s Five Year Forward View and Better Health for London – the report of the London Health Commission. Person centred care planning and supporting self-management of care are integral parts of Better Care Fund plans and initiatives such as the national Integrated Personal Commissioning programme are seeking to take this agenda further still.
As part of the London Health and Care Integration Collaborative’s events series, this timely session brings together colleagues from across the NHS and local government in London. It will enable you to hear from expert speakers and to learn about the latest developments in this area. Through a combination of plenary sessions and thematic surgeries, participants will have the opportunity to:

Hear examples of emerging innovative practice from London and elsewhere;
- Draw inspiration from the expert speakers, gain ideas to use in their own work context and share the experience of their own local practice;
- Explore how to make the most of the current policy enablers for greater patient, service user and carer involvement and control;
- Discuss barriers to greater patient empowerment and explore ways of overcoming these; and
Influence the focus and work programme of the London Health and Care Integration Collaborative.
