Each framework is accompanied by an FAQ document which is a quick way to learn what has changed from the current processes.
A key point is that from now on, SIs will not be classified as Grade 1 or 2 – they will all just be SIs. However, the level of harm will have to be recorded on STEIS so it will be possible to identify severe harm of death incidents – and they will still be categorised by type.
There are now just 14 types of Never Event – rather than 25 – so it will not be valid to compare rates of Never Event reporting pre and post April 2015.
