We are looking for experienced and energetic senior practitioner improvement manager to join our Care Close to Home Programme and support systems to improve Mental Health Delayed Transfers of Care (DTOCs).
This is an exciting opportunity to be part of a new Improvement Collaborative Programme working across LondonADASS, NHSE, and Healthy London Partnership to bring about real change for people with Mental Health problems. This role is crucial in providing understanding of key barriers and enablers in Mental Health Systems and providing hands on support to systems to engage in work to ensure that people are admitted to hospital are discharged in an effective and timely way and to their own home or usual place of residence wherever possible.
You will work with systems to support them in:
- Building up “Once for London” good practice, standards and improvement learning
- Local work to implement and measure best practice
- Awareness of and links with national support offers, to ensure London makes the best use of these
You will already have considerable experience of mental health systems and of working on complex programmes of change and using improvement collaborative methodology. You will also have experience and knowledge of the issues surrounding effective discharges in mental health settings, the metrics used to measure success and a good understanding of the interplay between stakeholders in acute, community, mental health, social care and wider local authority sectors.
The post holder will be expected to work closely with CCG, Local Authority and Mental Health providers for the completion of projects to time, budget and specification.
The post holder will be expected to engage professionally across health and care systems in London to ensure that projects are delivered in accordance with agreed methodologies and to uphold the highest professional standards. For more details please see attached job description.
